Personal Information We Collect from You via the “Contact Us” form
We collect the following personal information from you on the Worthing Shiatsu website “Contact us” forms:
- First and Last name
- Email address
- Additional health information if you choose to include that in your request for information or to make a booking
By entering this information, you are providing personal information that will be used by your practitioner, Matt Woods for the purposes of scheduling your appointment or returning your contact request and in order to ascertain whether Shiatsu treatment will be helpful for your heath needs. For legal reasons I am obliged to keep this information securely and privately for a period of a minimum of 7 years after treatments are discontinued.
Disclosure of Your Information
(i) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order.
(ii) in special cases, such as a physical threat to you or others. In the event that we are legally compelled to disclose your Personally Identifiable or Personal Health Information to a third party, we will attempt to notify you unless doing so would violate the law or court order.
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
What rights you have over your data
If you have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us.
